The Roadmap concept: using early on-treatment virologic responses to optimize long-term outcomes for patients with chronic hepatitis B

چکیده

Several large observational, longitudinal studies of the natural history of chronic hepatitis B (CHB) have demonstrated that high levels of hepatitis B virus (HBV) replication are associated with long-term risk of cirrhosis, decompensation, hepatocellular carcinoma, and liver-related mortality. The corollary is also true-profound and sustained suppression either spontaneously or during antiviral therapy will prevent disease progression and complications. Multiple analyses of various baseline factors and on-treatment responses have identified the absolute HBV DNA level after 24 weeks of therapy as the best predictor of long-term efficacy. Lower 24-week serum HBV DNA levels after lamivudine, telbivudine, or entecavir are associated with higher rates of maintained HBV DNA nondetectability, ALT normalization, HBeAg seroconversion, and lack of resistance. Patients with undetectable serum HBV DNA levels after 24 weeks have the best long-term outcomes while those with levels remaining above 10,000 copies per ml are unlikely to benefit from long-term therapy with that particular agent and either the addition or switch to another antiviral agent with increased potency but without cross resistance could be considered at this time point. In the future, improved on-treatment monitoring should facilitate treatment strategies to optimize long-term outcomes among patients receiving oral antiviral therapy for CHB.

EFFICACY AND SAFETY OF COMBINATION THERAPY OF INTERFERON-ALPHA 2B PLUS RIBAVIRIN FOR CHRONIC HEPATITIS C

Introduction: The aim of this study is to assess the therapeutic efficacy and safety of a particular brand of IFN alpha (PDferon B®) in combination with ribavirin (RlBA) on Iranian patients with chronic hepatitis C (CHC). The addition of RIBA to the standard treatment with interferon (IFN) alpha led to an improvement in sustained virologic response (SVR) from less than 20% with IFN monother...

Sustained Off-Treatment Response After Discontinuation of Long-Term Nucleos(t)ide Analogue Treatment in HBeAg-Seronegative Hepatitis B: A Case Series.

International guidelines recommend lifelong nucleos(t)ide analogue (NA) treatment in individuals with chronic hepatitis B (CHB) infection who are hepatitis B e antigen (HBeAg) seronegative, because hepatitis B surface antigen (HBsAg) seroconversion is rarely achieved. However, after terminating therapy, sustained responses and HBsAg loss have been observed. Clinical characteristics identifying ...

52-Week Efficacy and Safety of Telbivudine with Conditional Tenofovir Intensification at Week 24 in HBeAg-Positive Chronic Hepatitis B

BACKGROUND AND AIMS The Roadmap concept is a therapeutic framework in chronic hepatitis B for the intensification of nucleoside analogue monotherapy based on early virologic response. The efficacy and safety of this approach applied to telbivudine treatment has not been investigated. METHODS A multinational, phase IV, single-arm open-label study (ClinicalTrials.gov ID NCT00651209) was underta...

SAFETY AND EFFICACY OF INTERFERON ALFA FOR THE TREATMENT OF CHRONIC HEPATITIS C INFECTED SUBJECTS WITH TRANSFUSION DEPENDENT THALASSEMIA IN IRAN

Up to 30% of Iranian adult multi-transfused thalassemic patients are infected with hepatitis C virus (HCV) which can intensify the progression of liver disease caused by iron overload in this group of patients. Our aim was to assess the biochemical and virological response of interferon alfa (INF-α) and its safety in thalassemic patients with chronic HCV infection. This trial was a single c...
